General

Although the enclosed registration form can be used, participants are encouraged to register through the online system (click here). In case you don't want to register online the completed forms together with the appropriate payment should be sent to the Congress Secretariat. Each registration form is for one participant only and his/her accompanying person(s).

All amounts are payable in euro. Prices include 19% Czech VAT.

*Registration for students and nurses must be accompanied by a letter from the Head of the Department to prove their status.

Registration package

The registration package for the participants includes:

• admission to the scientific sessions and the exhibition area

• access to the opening ceremony

• abstract book

• final programme

• certificate of attendance

• coffee and tea during breaks

• welcome cocktail (Wednesday April 30, 2008)

Cancellation policy

All refunds will incur a 20% handling fee.

No refunds will be made after 1 March, 2008.

However, another delegate can be nominated as a replacement.

Cancellation must be in writing (by letter, fax, or e-mail) to the Congress Secretariat.

The date of postmark will be used when considering a refund. All refunds will be made after the Congress. Participants should state the bank details to which refunds should be made.

Confirmation

Upon receipt of the registration form(s) with the appropriate fees, confirmation will be sent together with a receipt of payment.

Once registered, registrations cannot be changed to another fee category (on request of the participant).

On site registrations

While it will be possible to register on site, we do suggest that participants take advantage of the lower fee (pre-registrations) and of the convenience of registering in advance.
